About this playbook

The HSA rule that landed on January 1, 2026 is the largest regulatory tailwind DPC has ever had. Five months in, almost no one's moved on it. In Massive Impact's independent research of the US DPC market, only 12.7% of practices mention HSA anywhere on their site (474 of 3,734 records, as of May 2026). In the same agency's audit of 150 DPC sites, only 5% put HSA on the membership page. The lead window is open.

This is a playbook, not an analysis. Each day is an artifact you can paste into your site, your front-desk binder, or your next employer email. No theory. No filler. Seven days, seven moves, done.

Who this is for

DPC owners, hybrid and pure model, who heard the HSA rule and meant to do something about it. Doctors who run the site themselves. Office managers who own member messaging. If you've got a membership page and it still doesn't say the word HSA, this playbook is for you.

A note on the caveats

The rule is real, but it isn't infinite. The limit is monthly, not annual. The cap applies to fees, not to every cash-pay service you sell. And eligibility specifics vary by HSA administrator. Every page in this playbook respects that. Honest copy converts; overclaim breaks the relationship before it starts.


Day 1 + Day 2

Day 1. The membership page sentence

Add one sentence above your pricing table. That's the whole task today.

The sentence (paste this):

As of January 1, 2026, your HSA dollars can cover the cost of your Direct Primary Care membership, up to $150/month for individuals and $300/month for families (per IRS Notice 2026-5). Eligibility specifics vary by HSA administrator, so your plan documents are the final word.

Two alternates if your page voice is shorter:

HSA eligible. As of January 1, 2026, your HSA can pay your DPC membership, up to $150/month individual or $300/month family (IRS Notice 2026-5). Check with your HSA administrator.

Pay your membership with HSA dollars. The 2026 IRS rule (Notice 2026-5) allows HSA reimbursement of DPC fees up to $150/month individual / $300/month family. Plan documents govern.

Where it goes. Directly above the pricing table on your membership page. If your pricing table sits on its own page, put it there too. If your homepage shows pricing, add it there as well. Three places, one sentence.

Why the sentence works. It's specific. It names the rule. It gives the cap. It tells the reader where to go next (their plan documents). It doesn't overclaim. A skeptical patient finishes reading and trusts you a little more than they did before.

Day 2. The patient-facing FAQ

Three Q&As, ready to publish. Drop them into your existing FAQ block, or start a new one if you don't have one yet.

Q1. Can I use my HSA to pay my DPC membership? Yes. As of January 1, 2026, HSA dollars can pay your DPC membership up to $150/month for an individual plan or $300/month for a family plan, per IRS Notice 2026-5. The exact reimbursement process depends on your HSA administrator, so check your plan documents.

Q2. Does my employer-sponsored HSA work the same way? Generally yes. The IRS rule applies to qualifying HSAs whether they're individual or employer-sponsored. Your employer's HSA administrator handles reimbursement under their plan documents. If you're not sure, ask your HR contact for the qualified medical expenses list.

Q3. What if I already paid out of pocket for 2026 months? Many HSA administrators allow reimbursement for qualifying expenses paid earlier in the same plan year. Save your receipts and ask your HSA administrator about a reimbursement request. The rule was effective from January 1, so months you've already paid may still qualify.

Caveat to include at the bottom of the FAQ block: The IRS rule covers DPC membership fees specifically, not every cash-pay service we offer. Per-month caps apply ($150 individual / $300 family), and eligibility specifics vary by HSA administrator. Your plan documents are the final word.


Day 3 + Day 4

Day 3. The employer pitch line + email opener

Open your CRM. Pull any HR contact from any employer conversation you've ever had, even the ones that stalled. Today's task is one sentence and a three-line opener you can send before lunch.

The employer pitch sentence (use it on your employer page, in talks, in emails):

As of January 1, 2026, your employees with HSA plans can reimburse up to $150/month of a DPC membership ($300/month family) under IRS Notice 2026-5. That's a tax-advantaged way to add primary-care access without adding a benefit line item.

The three-line email opener:

SubjectHSA + DPC, quick update for [Employer Name]

Hi [Name],

Quick heads up. The 2026 IRS rule (Notice 2026-5) now lets your team pay their DPC membership with HSA dollars, up to $150/month per employee. That changes the math on a primary-care benefit conversation we had a while back. Want to grab 15 minutes to walk through what it means for [Employer Name] this benefit year?

[Your name]

Why this works. No pitch, no PDF attached, no calendar link in the first email. One regulatory fact and a 15-minute ask. Employer benefits leads care about budget-neutral additions. The HSA rule is one.

Day 4. The front-desk script

Two sentences. That's the whole script. Print it, tape it under the front-desk counter, walk the front-desk team through it once.

When a patient asks at the counter or on the phone, "Can I use my HSA for this?":

Yes, the 2026 IRS rule lets you pay our membership with HSA dollars, up to $150 a month for individuals or $300 a month for families. We don't bill your HSA directly, so you'll reimburse yourself through your HSA administrator, and your plan documents are the final word on what qualifies.

Why two sentences and not three. A front-desk answer over the phone has to land before the patient interrupts. The first sentence gives the answer. The second handles the operational question that comes next (who processes the reimbursement). Anything more is a paragraph the team won't memorize.

For the Meet & Greet. If you do an in-person Meet & Greet, add one line to the standard intake: "If you've got an HSA, the 2026 rule lets it cover our membership, and we can show you how that works at the end of the visit." It signals you know the rule cold. Most practices don't.


Day 5 + Day 6

Day 5. The patient ROI math

Patients trust math they can do themselves. Add a short worked example to your membership page, right under the HSA sentence from Day 1.

The math callout (paste this):

Example: A $150/month individual membership is $1,800 a year. If you pay with pre-tax HSA dollars and you're in the 24% federal tax bracket, that's about $432 a year in tax savings versus paying with post-tax money. Your effective annual cost drops from $1,800 to roughly $1,368.

For example only. Your actual tax savings depend on your bracket, state taxes, and HSA administrator rules. Your tax advisor and plan documents are the final word.

A family-plan variant:

A $300/month family membership is $3,600 a year. At a 24% bracket, paying with pre-tax HSA dollars saves about $864 a year in federal tax. Effective annual cost: roughly $2,736.

Why this lands. Most members never calculate the tax-advantage value of HSA spending. The math is small, but it's real, and the patient does it once and remembers it. The "for example" framing is the honesty move. It earns the trust the number needs to do its work.

Day 6. The site audit checklist

Seven items. Check them on your site today. Each unchecked box is a 10-minute fix.

  • Membership page mentions HSA above the pricing table
  • Pricing page (if separate) mentions HSA in the same spot
  • FAQ block has at least one HSA Q&A (Day 2's three are a starting point)
  • Footer has a one-line tax-advantage note ("HSA eligible per IRS Notice 2026-5")
  • Blog or news section has a 2026 post about the HSA change (even a short one)
  • Employer page (if you have one) includes the Day 3 pitch line
  • Contact form options include "ask about HSA" as a topic dropdown

The benchmark. In Massive Impact's audit of 150 DPC sites, only 5% put HSA on the membership page. Three checked boxes on this list puts you ahead of nearly every site Massive Impact has reviewed. Seven puts you alone at the top of the field.


Day 7

Day 7. The publish + measure step

Push the changes live. Then start watching three numbers. None of them need a new tool. All three exist in the basic site analytics you already have.

What to track for the next 30 days:

  1. Form submissions that mention HSA. Add "HSA" as a topic option to your contact form, then count how many submissions select it. Baseline is probably zero. Watch it move.
  2. Time on the membership page. If the page mentions HSA above the pricing table, time-on-page should rise. Members are reading the new sentence. Note the baseline before you publish, compare 30 days after.
  3. Phone questions about HSA. Ask your front-desk team to keep a tally sheet for 30 days. One mark per HSA question. If it's higher than zero, the messaging is doing its job and the script (Day 4) is what they need.
What to do with what you learn

If form submissions mentioning HSA stay at zero after 30 days, the contact-form option isn't visible enough. Move it. If time-on-page didn't move, the sentence isn't placed where eyes hit it. Move it above the pricing table, not below. If phone questions spike, the script gets used.

Proof it works. The Tandem Health example.

Tandem Health (Maryland) added an HSA Q&A to their FAQ with the 2026 legislative clarification baked in. In Massive Impact's Authority Audit, Tandem was the only site of 15 to clear five or more lenses of best practice. The HSA Q&A is part of why. The lesson isn't that Tandem is unbeatable. It's that one practice doing one small operational thing right opens the door to operating ahead of the entire field. That practice can be yours by Friday.
